About the Indian Passport Documents Checklist, Fee and Annexures
Passport Seva asks for different papers for every kind of application: a fresh passport or a re-issue, an adult or a child, Normal or Tatkaal, a lost or damaged passport, a change of name or address, a Government employee or a Police Clearance Certificate. This Indian passport documents checklist asks the same questions as Passport Seva’s Document Advisor and builds one list for your case: the proof of address and date of birth it accepts, the Non-ECR proof, the old passport pages, the fee from the Passports Rules, the kind of police verification to expect and how the appointment works.
For the cases that need an annexure it also drafts it with your details: Annexure D (both parents’ consent for a minor), Annexure C (one parent has not consented), Annexure F (lost or damaged passport), Annexure J (joint photo declaration instead of a marriage certificate), and the Government employee’s Annexure A, G or H. The free preview shows the checklist and drafts as marked pages; a Pro pass unlocks the Word file, the PDF, the text and printing. Nothing is sent to Passport Seva or anywhere else.
How to use it
- Choose the service (fresh, re-issue or PCC), the reason for a re-issue, Normal or Tatkaal, whether the applicant is an adult or a minor, the date of birth and the booklet.
- Pick the proof of address and proof of date of birth you will take, your Non-ECR category and, for Government employees, the certificate your office gives you.
- For a child, choose whether both parents consent and fill in the parents’ details; for a lost passport, describe what happened. These go into the annexure drafts.
- Read the checks and the preview: the fee, the police verification and every document numbered. Then download the checklist and annexures as Word or PDF — with a pass, or after unlocking this result — sign the annexures and take them with the originals and self-attested copies.
Examples
Fresh · Normal · minor · born 14 June 2019 · address proof: parent’s passport · birth certificate · both parents consent
36 pages, valid for 5 years or until 18; fee ₹1,750 (₹1,575 after the 10% discount for children up to 8); Annexure D signed by both parents; generally no police verification when both parents’ passports name each other as spouse.
Re-issue · validity expired more than three years ago · Tatkaal · 36 pages · Non-ECR (class 10)
Tatkaal fee ₹5,000; old passport with copies of its first two and last two pages; any three documents from the Tatkaal list; the matriculation certificate if the old passport shows ECR status; Tatkaal applications go on post-police verification.
Re-issue · lost or stolen · Normal · 36 pages
Fee ₹5,000 (in lieu of a lost passport); the police report in original; proofs of address and date of birth; a copy of the old passport if you have one; Annexure F drafted with what happened. Tatkaal is not available for lost passports.
Common uses
- Getting the papers right before the PSK appointment, so the token is not refused.
- Applying for a child’s passport when one parent lives abroad or has not consented.
- Replacing a lost or damaged passport, with Annexure F filled in.
- Adding a spouse’s name without a marriage certificate (Annexure J).
- Government employees choosing between Annexure A, G and H.
Passport fees in India
Schedule IV of the Passports Rules, 1980 sets these fees, the same as the fee calculator on passportindia.gov.in (Tatkaal includes the normal fee):
- 36 pages, 10 years (18 and above, or 15 to 18 by choice): ₹2,500 Normal, ₹5,000 Tatkaal.
- 60 pages, 10 years: ₹3,500 Normal, ₹6,000 Tatkaal.
- Minors, 36 pages, 5 years or until 18: ₹1,750 Normal, ₹4,250 Tatkaal.
- In lieu of a lost or damaged passport: ₹5,000 / ₹7,500 (36 pages), ₹6,000 / ₹8,500 (60 pages); minors ₹4,250 / ₹6,750.
- Police Clearance Certificate: ₹750.
The rules give a 10% discount on the passport fee for fresh applications (not re-issues) of children up to 8 and people above 60. You pay online before booking the appointment, and the payment stays valid for one year.
Proofs of address and date of birth
- Address: any one of Passport Seva’s list — Aadhaar, rent agreement, electricity, water or telephone bill, gas connection, a photo passbook of a running bank account, Income Tax assessment order, voter ID, an employer’s certificate (public limited companies), a spouse’s passport, or a parent’s passport for minors. Only the present address needs a proof, but list every address of the last year in the form.
- Date of birth: for anyone born on or after 1 October 2023, only the birth certificate from the Registrar of Births and Deaths or the municipal corporation. For anyone born before, also a school leaving or matriculation certificate, PAN card, driving licence, voter ID, a Government service record or pension order, or an LIC or public company policy bond showing the exact date of birth.
Tatkaal: the extra documents and who cannot use it
Under Tatkaal, adults add any three of 13 identity documents (Aadhaar, PAN, voter ID, driving licence, ration card, student ID, a passbook with photo, the last passport and others) and minors any two of the first six, besides the address, birth and Non-ECR proofs, plus the Undertaking for Tatkaal Scheme. Tatkaal is not available for a lost or stolen passport, a passport damaged beyond recognition, any change of name, a change of sex, appearance, signature, date or place of birth, a change of parents’ or spouse’s name, or a minor’s application with Annexure C, among other cases. Tatkaal passports are processed on post-police verification and Tatkaal is not offered at Post Office PSKs.
Minors: Annexure C or D
Applicants under 18 are minors. Their passport is valid for five years or until they turn 18; those aged 15 to 18 may choose a 10-year passport instead, with the adult fee and Non-ECR documents. Both parents sign Annexure D and should come to the PSK, or one parent with both parents’ passports. When one parent cannot or will not consent — living or travelling abroad, divorced or separated, a pending custody case, deserted the family — the other parent signs Annexure C with the reason and its documents. Children under 4 bring a 4.5 × 3.5 cm photograph with a white background; older applicants are photographed at the PSK.
Police verification
Passport Seva’s FAQs describe three outcomes. No police verification for Government employees with an Identity Certificate (Annexure A) and, generally, for a minor’s 5-year passport when both parents hold passports naming each other and consent in Annexure D. After issue (post-police verification) for Tatkaal and for employees with a No Objection Certificate (Annexure G). Before issue (pre-police verification) in most other cases, including employees with a Prior Intimation letter (Annexure H). A re-issue more than three years after the old passport expired starts a fresh verification. The Passport Office decides the mode.
Limitations
- A checklist from Passport Seva’s published rules, not legal advice. The Passport Office may ask for more documents, and the official site prevails.
- For a change of name other than after marriage or divorce, a change of date or place of birth, adoption, surrogacy, J&K or Nagaland origin, or citizenship by registration, follow the Document Advisor on passportindia.gov.in: the checklist points there.
- Applications at Indian missions abroad have their own fees and procedures.
- It never fills in or submits the Passport Seva form and never books an appointment.

Frequently asked questions
What do I get without a pass?
Without a pass, Indian Passport Documents Checklist, Fee and Annexures shows every page with your details, the first part of the wording readable and the rest hidden, marked “MySmartCoPilot preview · not for use”. Until you unlock it, the result can’t be downloaded, copied or printed. A Pro, Premium or Ultimate pass, a one-time payment that never renews, unlocks the full result. The pricing page lists the passes and their prices.
What documents are required for a fresh passport in India?
A proof of present address and a proof of date of birth from Passport Seva’s lists, and the documents of a Non-ECR category if you qualify (for example the class 10 certificate). Minors add Annexure D signed by both parents (or Annexure C), married applicants who want the spouse’s name a marriage certificate or Annexure J, and Tatkaal applicants three more identity documents (two for minors). Take originals with one set of self-attested copies.
What is the passport fee for 36 and 60 pages?
In India: ₹2,500 Normal and ₹5,000 Tatkaal for a 36-page 10-year passport, ₹3,500 and ₹6,000 for 60 pages, and ₹1,750 and ₹4,250 for a minor’s 36-page passport valid until 18. A passport in lieu of a lost or damaged one costs ₹5,000 to ₹8,500. A Police Clearance Certificate costs ₹750.
Which annexure is needed for a minor’s passport?
Annexure D, the declaration signed by both parents or the legal guardian, in normal cases. When one parent has not given consent — because they live or travel abroad, the parents are divorced or separated, a custody case is pending or a parent has deserted the child — the parent applying signs Annexure C with the reason and its documents. Tatkaal is not available with Annexure C.
What do I need if my passport is lost?
Apply for a re-issue with reason “lost”: the police report in original, proofs of address and date of birth, a copy of the old passport if you have one, and Annexure F describing how and where it was lost. The fee is ₹5,000 for 36 pages (₹6,000 for 60). Tatkaal is not available for lost or stolen passports.
Is Aadhaar accepted as proof of date of birth?
Passport Seva’s current list of proofs of date of birth does not include Aadhaar: it lists the birth certificate, school leaving or matriculation certificate, PAN card, driving licence, voter ID, a Government service record or pension order, and a policy bond. Aadhaar is accepted as proof of address, and for Tatkaal as one of the identity documents.
