About the Affidavit Generator
Choose what the affidavit is for — change of name, address proof, date of birth, gap in education, income, marital status, loss of a document, or a general affidavit — and fill in the details. You get the standard Indian format: the deponent’s description, “do hereby solemnly affirm and declare as under”, numbered paragraphs that each begin with “That”, the deponent’s signature and a verification clause with place and date.
For a change of name it also prepares what the Department of Publication asks for to publish the change in the Gazette of India: the newspaper notice text, the Gazette matter to be signed in your old name with two witnesses, the soft copy for the CD, the undertaking and the hard-copy/soft-copy certificate — plus an optional generic deed of change of name. Everything prints with blank lines for anything you leave empty, so you can complete it by hand before the notary.
How to use it
- Choose what the affidavit is for. For a change of name, tick the documents you need.
- Enter the deponent — name, parent’s or spouse’s name, age, occupation and address — and choose affirm or swear.
- Fill in the details of the purpose, and add any additional statements, one per line.
- Add the place and date. Tick the stamp-paper option if you will print on stamp paper.

Common uses
- Change of name after marriage, for adding a surname or for spelling corrections, with the Gazette documents.
- Proof of address for a bank, school admission or a utility connection when other proof is not available.
- Date of birth, gap-in-education and income affidavits for admissions, scholarships and certificates.
- Single-status or marital-status affidavits, and affidavits for duplicate certificates after a loss.
How an affidavit is sworn
An affidavit is a written statement of facts made on oath or solemn affirmation. Oaths for affidavits are administered by courts, judges and magistrates, and by persons empowered by the High Court (for court cases) or by the State Government (for other affidavits) — the oath commissioners (Oaths Act 1969, s.3). A notary may also administer an oath and take an affidavit (Notaries Act 1952, s.8(1)(e)). Take the unsigned affidavit and your ID; you sign in their presence and they attest it. You may affirm instead of swearing — the form for affidavits covers both: “I do swear in the name of God / solemnly affirm that this is my name and signature (or mark) and that the contents of this my affidavit are true” (Schedule, Form No. 4).
Whether stamp paper is needed, and its value, depends on your State’s stamp law and the office that asked for the affidavit — check with them.
Only facts you know
Affidavits for courts must be confined to facts the deponent can prove from their own knowledge; statements of belief are allowed only on interlocutory applications, with the grounds stated (Code of Civil Procedure 1908, Order XIX rule 3). The verification here says the contents are true and correct and nothing has been concealed. If some paragraphs are based on information from others, tick that option: the verification then lists those paragraphs and the source.
False statements are a crime
A person bound by an oath who makes a statement they know or believe to be false, or do not believe to be true, gives false evidence (Bharatiya Nyaya Sanhita 2023, s.227). The punishment is imprisonment up to 3 years and a fine up to Rs 5,000, or up to 7 years and a fine up to Rs 10,000 if it is in a judicial proceeding (s.229). A false statement in a declaration that an authority must accept as evidence is punished the same way (s.236).
Publishing a change of name in the Gazette of India
Adults publish a change of name in the weekly Gazette of India, Part IV through the Department of Publication. Its guidelines for adults ask for:
- an advertisement of the change in a daily local leading newspaper, giving your father’s or husband’s name and address (send the original newspaper);
- an undertaking signed by you with your old and new name and particulars;
- the specimen matter, typed and signed in your old name with two witnesses, in duplicate;
- a soft copy of the matter in MS Word, without the witness portion and with your old name typed in place of the signature;
- two self-attested passport photographs and a self-attested photo ID;
- a certificate that the hard and soft copies are the same;
- a request letter with the fee paid through Bharatkosh.
The tool prepares the texts for the notice, the matter (both versions), the undertaking and the certificate. If the new name is from another religion and you are not changing your religion, say so in the undertaking. Minors, changes after marriage or divorce and changes of gender have separate guidelines — check the current requirements and fee on the Department’s website.

Limitations
- A generic format, not legal advice. Courts, embassies and some offices prescribe their own formats — use theirs if they do.
- It does not notarise, stamp or submit anything. You must swear the affidavit before a notary or oath commissioner yourself.
- The clauses are in English. Names and addresses typed in Hindi, Marathi or other Indian scripts appear correctly in the preview, DOCX and Print, but not in the PDF download.
- The deed of change of name is a generic format; Central Government employees and Indians abroad should use the specimen and attestation their office or the Indian mission asks for.

Frequently asked questions
Who can attest an affidavit?
A notary public, or a person empowered by the High Court or the State Government to administer oaths for affidavits (an oath commissioner); courts, judges and magistrates can also administer them. Sign in front of them, not before.
Does an affidavit need stamp paper?
It depends on your State’s stamp law and on the purpose, so ask the office that needs the affidavit what paper and value it requires. If you print on stamp paper, use the stamp-paper space option so the text starts below the stamp.
What is the punishment for a false affidavit?
Making a false statement on oath is false evidence under the Bharatiya Nyaya Sanhita: up to 3 years’ imprisonment and a fine, or up to 7 years in judicial proceedings (s.229).
Is an affidavit enough to change my name legally?
An affidavit records the change, and the newspaper notice and Gazette of India publication make it public. Which of these an office needs varies, so ask the office concerned. The tool prepares the texts for all three.
What is the verification at the end?
A short statement, dated and signed at a place, confirming that the contents are true and correct to your knowledge and that nothing material has been concealed. If some statements are based on information from others, it says which and gives the source.
Can I write the affidavit in Hindi?
