About the Salon & Beauty Client Consultation Form Maker

Make the consultation form your clients fill in before a treatment: your salon’s name and logo, the client’s details, the questions that matter for the treatment, a record of the allergy alert or strand test, the aftercare, the client’s agreement, permission for photographs, a privacy notice and signature boxes. Start from a template for hair colour, relaxers, perms and straightening, lashes and brows, waxing and threading, facials and peels, nails or massage, then change any question, add your own and remove the rest.

Questions can be Yes / No with a line for details, tick boxes, lines and boxes to write in, or a choice such as Light, Medium or Firm. Mark a test or treatment record For salon use. The print PDF has lines and boxes to fill in by hand; the fillable PDF has a form field on each one, to fill in on a tablet. Without a pass the form shows as a free preview; a Pro pass downloads both PDFs.

      How to use it

      1. Choose the treatment under Template and press Use this template.
      2. Type your salon’s name, address and contact details, and add your logo.
      3. Go through the Sections: change the questions to suit your treatments and the instructions of the products you use, add or remove items, and mark records your team fills in For salon use.
      4. Choose the privacy notice for your country and fill in how long you keep records, or write your own.
      5. The preview shows the pages as they print. Without a pass they are a free preview; with a Pro pass, download the print PDF or the fillable PDF.
      6. Save the form under My forms to make another one later; a backup moves your forms to another browser.

      Common uses

      • A hair salon asking colour clients about allergies and earlier treatments, and recording the allergy alert test.
      • A beauty therapist with one form each for waxing, lashes, facials and nails.
      • A spa that has massage clients fill in their intake form on a tablet before the treatment.
      • A salon owner updating the privacy notice on every consultation form at once.

Allergy alert tests and the hair dye warning

Oxidative hair colours sold in the European Union carry this warning on their label, set by the EU Cosmetics Regulation (Regulation (EC) No 1223/2009, Annex III): “Hair colorants can cause severe allergic reactions. Read and follow instructions. This product is not intended for use on persons under the age of 16. Temporary ‘black henna’ tattoos may increase your risk of allergy. Do not colour your hair if: you have a rash on your face or sensitive, irritated and damaged scalp; you have ever experienced any reaction after colouring your hair; you have experienced a reaction to a temporary ‘black henna’ tattoo in the past.” The hair colour template prints it, asks the questions it raises, and has a record of the allergy alert test for your team.

How and when to test is set by the instructions of the product you use: follow them, and change the template’s wording to match. To keep each client’s tests and colour formulas with their bookings, use the Salon & Spa Appointment Book and Client Register.

Privacy notices for the UK, the EU and India

      • United Kingdom and European Union: the notice says who is responsible for the information and how to reach them, why it is used and on what basis, how long it is kept, the client’s rights, how to withdraw consent and where to complain — the points of privacy information the ICO lists. Information about health is special category data, so the form asks for the client’s explicit consent with a tick box.
      • India: a notice written to sections 5 and 6 of the Digital Personal Data Protection Act, 2023: the personal data and its purpose, how to withdraw consent (as easily as it was given), how to raise a grievance and how to complain to the Data Protection Board. The Act asks for the option of the notice in English or a language of the Eighth Schedule to the Constitution, and for a parent’s consent for a client under 18. It covers personal data collected in digital form, and data collected on paper that is later digitised.
      • Anywhere else: a short general notice, or your own wording.

A notice is a starting point, not legal advice: check it against the law where you work.

Print or fill in on a tablet

The print PDF is for paper: clients write their answers and sign. The fillable PDF has a text field on every line and box and a tick box on every Yes, No and choice, named after its question, so a client can fill it in on a tablet or computer in a PDF reader, and you can keep the filled-in file. Both are on A4 or US Letter with your colour and font, and a heading never sits alone at the bottom of a page.

      Limitations

      • The templates are in English. Type your own wording in another language: Latin, Greek, Cyrillic and Devanagari print as text; other scripts are drawn by your browser into the PDF.
      • The templates are neutral starting points, not medical or legal advice: check every question against your treatments, your products’ instructions and the law where you work.
      • Forms are not collected online: print them, or fill in the PDF on a device.
      • Clients sign on paper, or in a PDF reader that can add a signature.

      Privacy

Your forms, your logo and your saved forms stay in this browser on your device (localStorage). The PDFs are made on the device. Nothing is uploaded.

      Frequently asked questions

What do I get without a pass?

Without a pass, Salon & Beauty Client Consultation Form Maker shows every page of the form as images marked “MySmartCoPilot preview · not for use”. Until you unlock it, the result can’t be downloaded. A Pro, Premium or Ultimate pass, a one-time payment that never renews, unlocks the full result. The pricing page lists the passes and their prices.

What does the free preview show?

The editor, the templates and your saved forms work in full, and the preview shows every page of the form as an image marked “MySmartCoPilot preview · not for use”. A Pro pass downloads the print PDF and the fillable PDF.

      Can I change the questions?

Yes, all of them. Every question, statement, heading and paragraph of a template is yours to change, move or remove, and you can add Yes / No questions, tick boxes, lines, boxes and choices of your own.

Is the form enough to make a treatment safe?

No form is. It records what the client tells you and the tests your team does; follow the instructions of the products you use and your own training, and do not go ahead when the answers or a test say otherwise.

Do I need a privacy notice on the form?

In the UK and the EU, clients have to be told how their information is used when you collect it, and India’s Digital Personal Data Protection Act asks for a notice with each request for consent. Choose the notice for your country, fill in how long you keep records, and check it against the law where you work.

Can clients fill it in on a tablet?

Yes: download the fillable PDF and open it in a PDF reader on the tablet. Every line, box and tick box is a form field; save the filled-in copy with the client’s name.
