      About the Patient Consent Form Maker (Bilingual, Print and Fillable PDF)

      Lay out a procedure consent form from your clinic’s own wording: the clinic and the doctor with the registration number, the patient’s details, the procedure, your sections — what the procedure is, its risks, the other options — the statements the patient ticks, and signature or thumb-impression boxes for the patient, a guardian, a witness and an interpreter, with the doctor’s own block. Dates and times are blank lines to fill in by hand.

      For patients who read another language, type the wording in a second language too and print it beside the English or after it: Hindi, Marathi and other Devanagari languages are sharp vector text; Tamil, Bengali, Urdu and others use your device’s fonts.       How to use it

      1. Type the clinic and the doctor — name, qualifications, registration number and council. Without them the form prints SAMPLE.
      2. Give the form a title and the procedure (or leave it blank for a line to write it on). Pick a second language if you need one and type its wording yourself.
      3. Under Sections, write your own text for each heading — what the procedure involves, its risks, the other options — and add sections as needed.
      4. Add the statements to tick in your clinic’s words, and choose the patient fields and the signature boxes (patient, guardian, witness, interpreter, doctor).

      What the tool does — and does not — write

      The form’s structure and labels come from the tool: the heading “Patient details”, Name, Age, Sex, Patient ID, Address, Procedure, Date, Time, Relationship to patient, Signature or thumb impression, Patient, Guardian (with the note “if the patient cannot sign”), Witness, Interpreter and Doctor, with Hindi and Marathi wording for them when you choose those languages, and the three section headings it starts with (Procedure, Risks, Alternatives), which you can rename. Everything else is yours: the description of the procedure, its risks and alternatives, and the statements the patient ticks. The tool gives no medical or legal advice and does not check your wording.

      Consent rules

      Rules on consent depend on your country, your council and the procedure. In India, for example, the Indian Medical Council (Professional Conduct, Etiquette and Ethics) Regulations, 2002 say that before an operation the physician should obtain consent in writing — from the patient, a parent or guardian for a minor, or a spouse as the case may be (clause 7.16) — and ask doctors to show their registration number on the papers they give patients (clause 1.4.1). Whether those rules apply to you, and what else your form needs, is for your council, your institution and your legal adviser to say: have the form reviewed before you use it.

      Print or fillable

      • Print PDF: the pages as they look in the preview, to print and fill in by hand.
      • Fillable PDF: the same pages with AcroForm fields over the blank lines and the tick boxes. Fill them in a PDF reader (Adobe Acrobat Reader, the browser’s PDF viewer, Preview) and print. Signatures and thumb impressions are made on paper — the boxes are not digital signature fields — and typing in the second language inside a field depends on the PDF reader.

      Limitations

      • MySmartCoPilot supplies no clinical or legal wording and does not review yours.
      • Signature boxes are for signatures and thumb impressions on paper, not digital signatures.
      • The fillable fields use a standard Latin font; text typed into them in another script depends on the PDF reader.
      • Lines in scripts without a bundled font use your device’s fonts and are images in the PDF; check them in the preview.
      • My forms are kept in this browser only; use the backup file to move them.

      Privacy

      Everything happens in your browser. What you enter or open here is not uploaded or stored by MySmartCoPilot. Downloads the form’s fonts from this site the first time you use them; your wording never leaves your device.

      What do I get without a pass?

      Why does my form say SAMPLE?

      The clinic’s name, the doctor’s name, the registration number or the council is missing. Type them under Clinic and doctor and the stamp goes away.

      Can the patient sign with a thumb impression?

      Yes: every patient, guardian, witness and interpreter box is labelled “Signature or thumb impression” and is large enough for either.

      Does the tool translate the form?

      No. You type the second-language wording yourself, so it says exactly what your clinic means. The form’s labels come in Hindi and Marathi when you pick those languages.

      Where are my forms saved?

      Under My forms, in this browser only; the form you are working on is kept in this browser too, so it is still there when you come back. Back up my forms downloads your saved forms as a file you can restore in another browser. Nothing is uploaded.

      Is the form legally valid?
