About the Prescription Pad Maker (Print-Ready, Bilingual)
Design a prescription pad in your own name: your name, degrees, registration number and council — always printed — your clinic, phone, timings and logo, a patient line (name, age, sex, date and fields of your own, all blank lines to write on), the ℞ symbol, a signature line and a footer. Choose A5, A4, half A4 across, A6, Half Letter, Letter or a custom size and one of three templates.
For a bilingual header, type your lines in a second language: Hindi, Marathi and other languages written in Devanagari are set as sharp vector text with the bundled Noto Sans Devanagari font, shaped so conjuncts and vowel signs come out right; Bengali, Tamil, Telugu, Gujarati, Urdu and others are drawn by your device’s own fonts as 1,200 dpi images. Make a press-ready PDF with bleed and crop marks for a print shop, or sheets of 2 or 4 pads for your own printer, with a calibration page to line the print up with pre-cut pad paper. Without a pass the pad shows as a free preview with its PDFs locked. Everything is made in your browser.
How to use it
- Type your name, qualifications, registration number and registering council. Without the number and the council the pad prints SAMPLE across the page.
- Add your clinic: name, address, phone, e-mail, timings and a logo, in the header or the footer.
- For a bilingual pad, pick the second language and type the lines in it (the tool does not translate). The patient labels come with Hindi and Marathi wording you can change.
- Choose the patient fields, the ℞ symbol, a blank, ruled or dotted writing area, an optional side column, the signature line and your footer.
- Pick the template, pad size, font and colours, and check the preview: it is the real PDF.
- For a print shop, choose the bleed and crop marks and press Press-ready PDF. For your own printer, choose the paper and pads per sheet, calibrate if your pads are pre-cut, and press Home-print PDF. Without a pass these PDFs stay locked in the free preview; Ways to unlock shows how to get them.

What Indian rules say a prescription shows
In India, the Indian Medical Council (Professional Conduct, Etiquette and Ethics) Regulations, 2002, published by the National Medical Commission, say:
- every physician displays the registration number given by the State Medical Council or the MCI in the clinic and on all prescriptions, certificates and money receipts given to patients (clause 1.4.1);
- only recognised medical degrees, and certificates, diplomas, memberships or honours that confer professional knowledge, go after the name (clause 1.4.2);
- the prescription letterhead shows the doctor’s name and designation in full with the registration particulars (clause 3.7.2);
- prescription papers carry the same details as the clinic’s sign board: name, qualifications, titles, speciality and registration number with the name of the State Medical Council (clause 7.13);
- a prescription makes clear when the physician dispensed any medicine himself or herself (clause 3.7.1) — write that on the pad when it applies;
- printing a photograph of yourself, or other publicity material, on a letterhead counts as self-advertisement (clause 6.1.2) — so use the logo space for a clinic emblem, never a photo of yourself.
That is why this tool always prints the registration line and stamps SAMPLE until it is filled in. What else your pad may show (address, timings, phone) and whether newer rules apply to you is for your council to say: check with it. Other countries have their own rules, and some medicines need official or tamper-resistant forms — this tool makes ordinary printed pads, not those.
Printing: a shop or your own printer
- Press-ready PDF: one pad per page with 3 mm or 0.125 in of bleed (backgrounds run past the edge so no white line shows after cutting), optional crop marks, and the PDF’s TrimBox and BleedBox set for the shop’s software. Colours are RGB; ask the shop whether it wants CMYK.
- Home-print PDF: 2 A5 or Half Letter pads per sheet, or 4 A6 or Quarter Letter pads, side by side with dashed cut lines, up to 50 sheets in one file. Print at 100 % / Actual size. Home printers cannot print the last few millimetres of the paper, so a colour band at the edge gets a thin white margin.
- Pre-cut pad paper: choose “Paper of the pad’s size” and set the same custom paper size in your printer’s settings. Print the calibration page, fold it in half both ways, and measure how far the printed cross is from the folds; enter that as the offset and every pad lines up. The offset is kept for that paper in this browser.
Languages and fonts
English, and other languages in the Latin, Greek and Cyrillic alphabets, are real, selectable text in the font you choose (Noto Sans, Inter, Montserrat or Poppins, all under the SIL Open Font License), embedded in the PDF. Devanagari lines (Hindi, Marathi, Nepali, Sanskrit, Konkani, Maithili) are shaped in your browser with Noto Sans Devanagari and drawn as vector outlines, so the shop needs no font and conjuncts such as क्ष, त्र and the reph print correctly. Lines in Bengali, Assamese, Gujarati, Punjabi, Odia, Tamil, Telugu, Kannada, Malayalam, Sinhala, Urdu and Arabic use your device’s own fonts and go into the PDF as images of 1,200 dpi (a little less for a line longer than about 17 cm): sharp in print, but not editable — check them in the preview.
Limitations
- MySmartCoPilot adds no medical wording: no drug names, doses, advice or certificates. The pad is a layout for your own handwriting.
- No signature picture can be added and no date is printed: the signature and the date are blank lines.
- There is no PNG or “e-prescription” export, and no medical, sick-leave or fitness certificate templates.
- Colours are RGB. The tool does not make CMYK or spot-colour (Pantone) files.
- Lines in scripts without a bundled font depend on your device’s fonts; if the preview shows boxes instead of letters, use another device or browser.
Privacy
Everything happens in your browser. What you enter or open here is not uploaded or stored by MySmartCoPilot. Downloads the pad’s fonts from this site the first time you use them; your details never leave your device.

What do I get without a pass?

Why does my pad say SAMPLE?
Because the registration number or the council is missing, or the number has no digits. Type both under Doctor and the stamp goes away. A pad without registration particulars should not be used for prescriptions.
Can I print the pads on my own printer?
Yes: choose the paper in your printer and the pads per sheet, and print the home-print PDF at 100 %. Two A5 pads fit on an A4 sheet and four A6 pads on one sheet; cut along the dashed lines. Without a pass the PDF stays locked in the free preview.
My pads are already cut. How do I line the print up?
Choose “Paper of the pad’s size”, download the calibration page and print it on one of your sheets. Fold the sheet in half both ways, measure how far the printed cross is from the folds, and enter that distance under Calibrate my printer. The offset is saved for that paper in this browser.
Will the Hindi or Marathi print correctly at the print shop?
Yes. Devanagari lines are shaped in your browser and drawn as vector outlines, so the shop needs no font. Check the preview: it is drawn from the same PDF.
Are my details uploaded or kept?
Nothing is uploaded: the PDFs are made in your browser. If “Remember my details in this browser” is ticked, your details stay in this browser’s storage for next time; untick it or press Clear to remove them.
