You inherit a programme whose risk register has 80 entries. Half have not been touched for three months, many read like "resourcing" or "technical issues", and the steering group no longer looks at it. How do you make it useful again?
- What it tests
- Cleaning a register without losing real risks, rewriting entries so they can be owned and watched, and building a review routine that people use.
- First step
- Sort the 80 entries into live risks, issues that have already happened, risks that can no longer happen and topics that are not risks yet, before rewriting anything.
- Follow-ups
-
- What do you do with an entry such as "resourcing"?
- How often would you review the register, and with whom?
- How do you get the steering group to look at it again?

Your app needs another team's payments API, which they say "should" be ready in week 6. Your own integration work starts in week 7. Where does this go in the RAID log, and what do you do about it?
- What it tests
- Telling a dependency from a risk and an assumption, writing the risk the dependency creates, and agreeing evidence and dates with the other team instead of hoping.
- First step
- Record the dependency with its owner and need date, then ask the other team what has to happen for week 6 to hold and what would show early that it will not.
- Follow-ups
-
- Why is "should be ready in week 6" more than one entry?
- What trigger would you agree with the other team?
- What would you plan in case the API is late?
